Description On December 6, 2005 at about 2035 (local time ), while delivering intermediate fuel oil (IFO) to the ship SEA SYNERGY, personnel aboard the tank barge INVESTIGATOR heard oil splashing down on the deck of the barge from the side of the ship and shut down the transfer. This document captures the lessons learned from that incident which took place near Vancouver, Washington.
