Description Anyone who wants to own or operate a facility where dangerous waste is treated, stored, or disposed must obtain a "Dangerous Waste Management Facility Permit" (commonly called a hazardous waste management facility permit or a TSD permit). There are two parts to a Dangerous Waste Permit Application - Part A and Part B. This booklet provides Part A Form and the instructions for completing the form.
