Description | To fill out this form on-line see "related web content below". This form is used to report an event or casualty that happened to a vessel while in transit in Washington waters. The information in this report will be used by the Department of Ecology, Spills Program Marine Information System where it will analyze causes and contributing factors relating to vessel casualties. |
