Description Dental offices generating wastewater subject to new federal rules at 40 CFR part 441 must submit a one-time compliance report by October 12, 2020 (or within 90 days of startup, if after July 15, 2017). This form was developed to allow ‘Dental Dischargers’ to satisfy that report requirement and all follow-up reporting required by that rule. It includes all requisite content for that report and report updates.
NOTES Only Dental Dischargers outside of delegated pretreatment program service areas are to complete this Ecology form. Dental Dischargers served by municipalities implementing a pretreatment program would follow the instructions of their pretreatment Control Authority. Contact person current on this page March 2023.
