| Description | This form is required for businesses reporting an emergency chemical release under the Emergency Planning & Community Right-to-Know Act. In addition to immediate verbal notification, submit this report within 30 days following release of: • Reportable quantities of Extremely Hazardous Substances listed in 40 CFR 355, Appendix A and B. • Chemicals that require release reporting under section 103(a) of the Comprehensive Environmental Response, Compensation, and Liability Act (CERCLA). |
